October 10, 2013
Burlington, MA –Lahey Health and Honoring Choices Massachusetts announced today that Lahey Health has become the healthcare system Community Partner of Honoring Choices Massachusetts. The partnership will incorporate the Honoring Choices model in the clinical settings at Lahey Health’s acute care hospitals and affiliated provider and physician groups.
The goal of the program is to support Lahey Health clinicians in facilitating healthcare planning discussions with patients and their families, and in offering quality, personalized care throughout patients’ lives. Clinicians at Beverly and Addison Gilbert hospitals; Lahey Medical Center, Peabody; Lahey Hospital & Medical Center, Burlington and all Lahey Health community group practices will work in collaboration with Honoring Choices staff to develop the appropriate educational approach for patients and families as it relates to advance care planning.
“Clinicians throughout Lahey Health have long honored and respected the healthcare choices of the patients and families we are privileged to serve,” said Gregory Bazylewicz, MD, chief network development officer for Lahey Health. “Partnering with Honoring Choices Massachusetts will further our mission of delivering high-quality, high-value healthcare. This will also foster patient-clinician dialogue about the patient’s healthcare plan, ensuring that the patient and his or her wishes remain at the center of all that we do.”
“I commend Lahey Health for their commitment to furthering the dialogue and working in partnership with their patients to ensure that patient choices and preferences for care are well defined and carefully honored,” said Ellen DiPaola, president and co-director of Honoring Choices Massachusetts. “Each of our Community Partners is committed to providing its patients and patient families with resources to be better informed, to make a personal healthcare plan, and to have greater access to quality, coordinated care in the community.”
